18th International Conference on Integrated Care (ICIC18)

The International Foundation of Integrated Care (IFIC) in partnership with RIVM and Vilans presents the 18th International conference on Integrated Care “Value for People and Populations: Investing in Integrated Care” to take place in The NBC Congrescentrum, Utrecht, Netherlands 23 – 25 May 2018.

The conference will bring together researchers, clinicians and managers from around the world who are engaged in the design and delivery of integrated health and social care.

Implications of interprofessional primary care team characteristics for health services and patient health outcomes: A systematic review with narrative synthesis

Interprofessional primary care (IPPC) teams are promoted as an alternative to single profession physician practices in primary care with focus on preventive care and chronic disease management. Characteristics of teams can have an impact on their performance.

However, the empirical evidence of the implications of IPPC team design on various aspects of care processes or health outcomes is specific to particular contexts, but a general understanding of optimal team design is not available.

The reimagination of sustainable integrated care in Ontario, Canada

To encourage clinical and financial efficiency, the Canadian province of Ontario initiated an integrated care program – Integrated Funding Models (IFMs) that required collaboration and coordination across acute and post-acute care sectors. This research shows how program implementers went beyond policy-makers’ original designs, to make integrated care sustainable for chronic diseases.
